The core building blocks of good health, like restful sleep, maintaining a balanced diet, staying active, and keeping medical appointments, rely heavily on strong executive function. But for people with ADHD, this doesn’t always come easily.
While ADHD is often discussed in relation to school or workplace performance, its impact on physical well-being can often be overlooked. Whereas the reality is that ADHD can put a silent, systemic strain on daily health.
To help us understand these impacts better, we ran a survey asking people living with ADHD about how the condition affects their physical health and self-care.
Our main findings were that:
91.2% said that ADHD has negatively affected their overall health
92.4% said that ADHD actively impacts how well they sleep
85.2% said that ADHD makes their daily diet worse
78.8% said they’d either completely missed or been late to a healthcare appointment due to ADHD
72.8% said ADHD stops them from exercising as much as they would like to
The survey, which ran for exactly two months from late April to late June, collected 250 responses from adults living with ADHD. Like our previous surveys, it was promoted in partnership with the ADHD Chatter Podcast, presented by Alex Partridge.
Here are all the details on the findings:
ADHD and sleep
How does ADHD affect sleep quality?
We asked:
92.4% is considerable, suggesting that the vast majority of people with ADHD struggle with their sleep.
Some of this might stem from dopamine regulation, as Danielle Mulligan, our Nurse Independent Prescriber, explains:
‘In people with ADHD, dopamine, a chemical in the brain, can become dysregulated – and this plays a critical role in sleep. Usually dopamine levels rise in the morning, helping us to feel more awake and alert, then decline in the evening, making way for melatonin – the sleep hormone – and we’ll start to feel tired and ready to rest. Because dopamine levels can be more erratic in people with ADHD, this cycle can become disrupted – so someone might feel awake and alert later in the evening, and not be as able to get quality restful sleep.’
A consequence of this, particularly for those with regular office hours who need to get up early every morning, is that sleep deprivation can be common.
Another common ADHD trait is hyperfixating on thoughts or tasks when there are fewer distractions, which tends to be late at night. So racing thoughts could be another reason why someone with ADHD might find it difficult to get enough sleep.
And lastly, certain conditions that interrupt sleep, like restless legs syndrome, are also more common in people with ADHD.
In order to tackle the sleep problems associated with ADHD, Danielle recommends following good sleep hygiene practices, like:
Limiting caffeine, particularly after 2pm
Avoiding screens for at least an hour before bed
Making a to-do list of all the things you need to remember for the next day before going to bed, so it isn’t weighing heavily on your mind while trying to wind down
Eating your evening meal at least three hours before going to bed
Avoiding sugary foods or drinks before bed
Making sure your sleeping environment is cool, dark, quiet, and comfortable
And ‘while good sleep hygiene won’t eliminate the underlying cause of sleep problems associated with ADHD,’ she says, ‘it may help to lessen the effects of poor sleep and, in turn, reduce the exacerbating effects of tiredness on ADHD symptoms.’
Do people with ADHD get the right amount of sleep?
We also asked:
The recommended average sleep for adults is 7-9 hours per night, however, around two thirds (66.8%) of respondents said they averaged below this. As well as helping to cause sleep problems, lack of sleep can also worsen ADHD symptoms, creating a difficult cycle that’s hard to get out of.
Not getting enough sleep can:
Worsen inattention
Increase restlessness
Hinder emotional regulation
Make certain ADHD medications less effective
Following good sleep hygiene, in addition to creating a more consistent routine and avoiding taking certain ADHD medications too late in the day, might help to improve sleep quality and consistency.
As Danielle explains: ‘This result doesn’t come as a surprise. Sleep is a massive factor for people with ADHD. While ADHD symptoms themselves can make consistent and restful sleep more difficult, not getting enough of it can in turn worsen symptoms, so it creates an unforgiving cycle that’s tough to break out of.
‘Another thing to keep in mind is that people with ADHD might have lived with poor sleep for a long time, and not really noticed that it’s an issue because their symptoms might mask the effects of it. For example, if a neurotypical person doesn’t feel as alert or focused on a task on a given day, they might be able to more easily identify a poor night’s sleep as being the cause. For a neurodiverse person, they might be more likely to encounter problems with focus and concentration, so they might not realise the effects of a bad night’s sleep as readily, and therefore not see that poor sleep could be making their symptoms worse.
‘So for someone with ADHD – who may already have a circadian rhythm that’s shifted slightly later anyway, and may be dealing with racing thoughts or restlessness while trying to sleep – this could mean that broken sleep takes longer for them to notice and fix, because it’s potentially easier for them to miss the problems it may be causing.’
Does ADHD affect diet?
We asked:
These findings suggest that, for most people with ADHD, the condition has an adverse affect on their diet.
There are several reasons why ADHD can lead to poor dietary choices and disordered eating. On the one hand, symptoms like hyperfocus can cause someone to fixate on a task and, consequently, forget to eat. But on the other hand, ADHD symptoms can also trigger someone to overeat. This can happen when dopamine levels dip during the day, causing you to snack when you’re not really hungry, because of the quick hit of dopamine it provides.
‘Interoceptive blunting’ is another symptom that some people with ADHD experience, which means a reduced ability to consciously detect, interpret, or integrate internal bodily signals. So when it comes to diet, it can affect how well someone is able to recognise hunger or thirst cues. If this happens, by the time they come to eat, they might be so hungry that they choose the quicker and easier option (which often isn’t the healthiest) rather than taking the time to prepare food from scratch.
Sensory processing differences can be another reason why someone with ADHD might have a complicated relationship with food. If certain food textures and smells cause sensory overload, you’re more likely to avoid them, making you more selective about what you eat.
This can cause them to be less adventurous and experimental with a variety of foods, like fruits and vegetables for example, and instead choose familiar, blander, or in some cases heavily-processed options, leading to reduced nutrient intake.
‘Again, this result isn’t completely unexpected.’, says Danielle. ‘ADHD can cause symptoms that push people into making food choices in a hurry, and it’s in this scenario where people often go for the quicker, less healthy option.
‘If you’re pushed for time or just really hungry, that ready-meal or ultra-processed item might seem more appealing than opening the cupboard and making something from scratch.
‘On the other hand, it is encouraging to see that 14% of respondents said that ADHD didn’t make a difference to their diet, or pushed them to eat in a more healthy way – which illustrates how ADHD can vary from person to person.
‘In some instances, someone with ADHD may be able to ‘channel’ their hyperfocus into diet planning, or hyperfixate on the nutritional values of what they’re eating, ultimately leading them to eat more healthily. And in some cases, people with ADHD might find the crunchier textures associated with vegetables like carrots, or the grainy texture of legumes more satisfying from a sensory perspective.’
Does ADHD affect health appointment attendance?
We asked:
These findings show that symptoms of ADHD can also affect health in less direct ways, such as missing or deferring important check-ups.
This is because common ADHD symptoms include struggling with organisation and time management, even for important things like medical appointments. Meaning that time management and scheduling aren’t just minor inconveniences for people with ADHD, they’re major barriers to accessing care.
If you combine the people who have completely missed an appointment with those who’ve been late, you get a staggering 78.8%.
This means that nearly 4 out of 5 respondents have had their medical care disrupted in some way due to their ADHD symptoms, highlighting that time blindness and executive dysfunction are pervasive, systemic issues for people with ADHD.
Missing a doctor’s appointment also often carries financial penalties, delays in care, and can strain the patient-doctor relationship. And even for the 25.2% who did make their appointments but showed up late, there can still be consequences. Chronic lateness can cause stress, anxiety, and shortened appointments.
However, it’s also worth highlighting the 21.2% (roughly 1 in 5) of respondents who managed to keep their medical schedules completely unaffected by their condition. This could be due to milder symptoms, highly effective coping mechanisms, or a strong external support system (like a partner or parent managing their schedule).
Regardless, it’s clear that the traditional healthcare system, which relies heavily on patients remembering dates, tracking time, and arriving precisely on schedule, is a little unfriendly to the ADHD brain.
For healthcare providers, this result underscores the need for accessible accommodations, such as text reminders on the day of the appointment, flexible grace periods, or easier rescheduling options. For individuals with ADHD, it validates the fact that if you struggle to make it to the doctor on time, you’re absolutely not alone.
As Danielle explains: ‘Out of all the questions we’ve asked in this survey, this one gives the most sobering insight into how ADHD can affect wider health. Over half of respondents said they’d missed a medical appointment due to ADHD, which could potentially have an impact on identifying and treating health conditions of varying severity.
We haven’t surveyed a separate pool of neurotypical people here, so it’s difficult for us to draw comparisons based on this data alone. But a piece of research that came out of the University of Bath found that people with ADHD were 60-90% more likely to miss a doctor’s appointment compared to the general population.
Time-blindess and executive dysfunction are the obvious factors to discuss in this context. But anxiety, which many people with ADHD struggle with, could be playing a role too. It’s common for people to feel nerves or dread before a medical appointment, and for people with ADHD this might feel amplified for a number of reasons – for example hyperfixating on the possible negative outcomes of tests, or anticipatory anxiety leading to ‘task paralysis’.
It’s a difficult thing for patients to overcome alone, and is a factor ADHD practitioners need to keep in mind when managing a patient’s care.’
Does ADHD affect physical activity and exercise?
We asked:
This result highlights a classic component of ADHD: the gap between intention and action. While exercise is widely known to drastically improve ADHD symptoms by boosting dopamine and norepinephrine, these numbers show that the condition itself often acts as a barrier to it.
Nearly 3 out of 4 respondents (72.8%) want to exercise more but find that their ADHD actively stops them. This high percentage points directly to some core ADHD challenges:
Executive function and task initiation: For a neurotypical person, going to the gym is one step. But for someone with ADHD, it can be a paralysing sequence of steps (find clothes, pack a bag, travel there, transition between tasks, and so on).
Delayed gratification: ADHD brains crave immediate dopamine. While exercise often feels good afterwards, the effort required to start doesn’t offer an immediate reward, making it harder to get started.
Routine disruption: People with ADHD often struggle to maintain consistent good habits. Missing a single day can completely break a fitness routine for weeks or months.
But on the flip side, 11.2% of respondents say that ADHD actually helps them to exercise more. This smaller group could be leveraging specific ADHD traits to their advantage.
If someone with ADHD finds a sport or workout they genuinely love, for example, it can become a hyperfixation, leading to increased engagement with it. And for those with the predominantly hyperactive or combined-type ADHD, intense exercise can serve as a release valve for excess energy. And lastly, some people with ADHD intuitively learn that exercise helps with their symptoms, and so use exercise as a daily tool to manage their ADHD.
As Danielle explains, ‘I think an external perception many people have of ADHD is that the hyperactivity element would spur on people with the condition to exercise more. And this is the case for some people (as we saw here with 11% of respondents). But almost three quarters of respondents said that ADHD stopped them from exercising as much as they’d like to. And this might be due to several reasons.
For many people, exercise is something they do once they’ve got work and domestic responsibilities out of the way for the day. If someone is working hard at a task or doing chores at home, then time-blindness can eat into the spare time they have available to exercise.
Executive function can also be a factor. Someone might feel like they don’t have the motivation to start exercising, because of all the conflicting information out there about the various ‘best’ methods and where to begin. Someone new to exercise researching this might experience information overload, feel overwhelmed, and come to a point of task paralysis.
And once again, dopamine is a critical element here too. Dysregulated levels can have an impact on energy throughout the day, meaning someone may not feel physically up to exercise at the specific time they’ve allotted for it.
Some people with ADHD take an ‘all-or-nothing’ approach to tasks, and so hyperfixate on perfection, and this can impact follow-through too. If their allotted time for exercise comes up a little short, or the conditions aren’t right for it (for example the gym is too warm or a favourite pair of running shoes need a wash), someone might feel that the exercise session isn’t going to be their best effort – and forego the session.’
How does ADHD affect overall health?
We asked:
While this result might seem dramatic at first, for anyone who has lived with ADHD or understands it well, it’s unlikely to be surprising.
A whopping 91.2% of respondents reporting a negative impact on their overall health proves that ADHD is not just a ‘focus problem’ or an issue related to school or work; it’s a full-body, full-life health challenge.
When you connect this to the rest of the data above, the picture becomes clear. When over 9 out of 10 people state that a condition harms their overall health, it means the negative impact is systemic:
Chronic stress: living in a world not built for ADHD can create constant, low-level stress (worrying about missed deadlines, lateness, or social faux pas). Chronic stress affects the immune system, digestion, and cardiovascular health.
The domino effect: Executive dysfunction makes basic self-care incredibly difficult. This includes ‘revenge bedtime procrastination’ leading to chronic sleep deprivation, forgetting to eat (or binge-eating for quick dopamine), and forgetting to drink water or take medications.
Co-occurring mental health conditions: The constant friction of living with ADHD can often trigger or worsen anxiety, depression, and burnout, all of which directly degrade physical health.
And yet, 7.2% of respondents said that ADHD has no effect on their health, and 1.6% said it has a positive effect.
Those who said it has no effect might view their ADHD strictly as a cognitive quirk that affects their work or organisation, but don’t feel it has bled into their physical vitality. They might also have a highly protective environment or scaffolding that helps keep their health on track.
And those who said ADHD improves their health are likely to be people who have channelled their hyperfocus into health-conscious lifestyles (like nutrition or competitive sports) or feel that their high energy levels keep them physically vibrant.
Overall, this result can be read as a clear call for a shift in how the medical community views ADHD. It can no longer be treated as a minor behavioural inconvenience, as Danielle explains:
‘Ultimately, these findings show that we need to highlight the profound effects that ADHD can have on overall health. While ADHD specialists are used to seeing and managing the secondary impacts of it (like problems with sleep or diet), it’s important that primary care and GPs take this into account as well.
‘It’s also important to keep in mind that when a patient with ADHD books an appointment with their GP, their neurodivergence doesn’t just switch off. The executive dysfunction and time-blindness are still there, acting as barriers to their care. To take one example, it’s surprising how many general practices still lack basic, automated safety nets like text appointment reminders, which can mean the difference between a patient receiving immediate support or slipping through the cracks.
‘True neuro-inclusive healthcare means the whole system adapts. By implementing simple, accessible accommodations at the primary care level, we can help to ensure that having ADHD becomes much less of a barrier to maintaining your physical well-being.’
